Prise en charge des interruptions médicales de grossesse à la maternité du centre Aliénor d'Aquitaine au CHU de Bordeaux : le point de vue des patientes

Abstract : Introduction: termination of pregnancy’s medical support is complex and multidisciplinary. It requires support and attentiveness from the whole medical staff. The care strategy also aims to preserve fertility and the later obstetrical prognosis of patients. The goal of this study is to evaluate the global satisfaction level of the patient taken in charge at the maternity ward of Bordeaux University Hospital for a medical termination of pregnancy in order to upgrade the process in fetal medecine care. Material and methods: monocentric prospective and descriptive study at the maternity ward of Bordeaux University Hospital including every patients who have had a medical termination of pregnancy at a term superior to 15 weeks of gestation between the 2nd of January, 2018 and the 28th of January, 2019 under the agreement of ethical research committee. Two questionnaires were distributed : the « initial » one during the hospitalisation and the « post termination of pregnancy » one during the consultation three months later. Results: during the study, 36 patients answered to the initial questionnaire, 32 patients answered to the post termination of pregnancy one and 28 patients answered to both of them. The average maternal age was 32 years old, the termination of pregnancy’s average term was 24 gestation’s weeks. In the initial questionnaire, 72.2% of the patients are very satisfied by the global patient care and according to the post operation questionnaire 59.4% of them are very satisfied. The major positive point highlighted is the healthcare workers' professionalism and empathy. The pluridisciplinary aspect with a good coordination between professionals is an important asset. The negatives aspects they pointed out are the fact that the premises are not very welcoming, the insufficient presence of the psychologist and the absence of a consultation room with the anaesthetist. A majority of the patients (86.1%) wanted to see the fetus in the delivery room. This encounter could enable them to make their e nd of pregnancy concrete, but also to initiate and help their grieving process. Conclusion: patients treated for a termination of pregnancy at the maternity ward of Bordeaux University Hospital are very satisfied all along their follow-up. Psychologic support needs to be constant and adapted to each couple. Psychologist presence, premises adaptation and anaesthetic care are axis for improvement that are brought to light.
