Acceptation et limites de la réorientation des patients « non urgents » se présentant aux Urgences vers des structures de médecine générale

Abstract : Introduction: overcrowding in emergency department is partly due to the use of patients who consult for “non urgent” problems. It has been proposed the possibility of reorienting these patients to alternative structure with general practitioner, after the patient's agreement. The purpose of the study was to evaluate the acceptance of reorientation of nonurgent patients during working hours, and to collect the reasons for refusal or acceptance. Methods: we carried out a monocentric observational prospective study in the Emergency department of Agen. Results: 150 questionnaires were collected, with a population including a predominance of women (53%), with a mean age of 34 years. Most patients came on their own or their family's, 49% for pain or a major concern, 46% because their doctor could not receive them. The reorientation acceptance rate is 79% (n = 113), favored by the proximity of the place of consultation and the short delay. Refusals are mainly related to the wish to access to a specialist or the availability of medical services like imaging or laboratory tests. Discussion: the reorientation is one of the tracks that can lead to decrease the overcrowding of Emergency department. Adjustments are needed to facilitate patient adherence.
