Dépistage des troubles visuels chez l’enfant de 0 à 3 ans en médecine générale : élaboration d’un outil par méthode Delphi

Abstract : Background: the general practitioner is the main actor in the follow-up of the child. Vision screening is an integral part of this follow-up, but it seems to be little done in practice. Aim : We wanted to create, based on the recommendations, a clinical screening tool for visual disorders in children aged 0 to 3 years old, adapted to primary care, based on expertise of general practitioners. Method: according to the Delphi technique, we evaluated the degree of agreement between 20 experts per rating scale, during 2 rounds of questionnaires. Proposals that didn’t reach consensus and those deemed appropriate with dissenting comments, were reworded in the second questionnaire. The final consensus was reached in the 2nd round following the stability of the results. Results: a final consensus among the experts was reached on 51 out of 62 proposals. These results suggest that most of the recommended tests are applicable in general practice. Secondary analysis of the comments identified the main barriers to conducting the screening, including lack of knowledge, lack of time, and lack of equipment to conduct the screening. Conclusion: based on consensus and expert comments we have developed a clinical vision screening tool in children aged 0 to 3 years old, intended for general practitioners. This tool includes a simple and accessible memo sheet, a notice and a list of necessary equipment. We assume that the widespread dissemination of such a tool will facilitate screening. Future research would assess the applicability of this tool in general practice and its impact on primary care screening practice.
