Intérêt d’un protocole d’harmonisation des pratiques au sujet de la lombalgie aiguë commune, dans un pôle de santé interprofessionnel à Saint-Martin-d’Hères : enquête par entretiens semi-dirigés auprès de médecins généralistes

Abstract : Background: Acute and chronic low back pain have a large prevalence in primary care, with major human and socio-economic consequences. Confronted with this problem, a community health center created and set up, in 2018, an interprofessional protocol whose objective was to facilitate the harmonization of practices and improve the case management of each patient. Objective: The main objective of this study was to assess the use of the « low back pain protocol » by general practioners (GP). The second objective was to understand and explore the different obstacles impeding the use of the protocol by certain GPs. Method: Qualitative study by semi-structured interviews of ten GPs, between October and December 2019.Results: Despite variable usage, all of the GPs assimilated the practice guidelines. The protocol enabled some GPs to confirm their practice. It allowed others to improve their diagnosis and therapeutic process. Globally, the protocol helped attune interprofessional communication, coordinate the care of patients, harmonize the practice of GPS and standardize the exchange between the GPs, the physiotherapists, and the patients. The main obstacle to the assimilation and use of the protocol, was the GPs’ lack of detailed knowledge of the protocol. Also, the initial complexity of the protocol, the lack of time, the lack of coverage and the many projects in the health center, were identified as obstacles. Conclusion : Despite the difficulties encountered during the interprofessional protocol‘s implementation, all GPs mentioned its overarching qualities. One could suggest that these qualities may be transposed to other protocols, regardless of their subject. Furthermore, it would be interesting to study the perception of the patients regarding the effect of the protocol.
