Characteristics of the senior pre-travel consultation: a descriptive study at a travel clinic in Bordeaux, France (2013 to 2018)

Résumé

Background : Older travelers have socio-medical characteristics insufficiently taken into account during his pre-travel consultation. The aim of this study is to describe the vaccination and malaria prevention practices during the senior pre-travel consultation. Method : A cross sectional study was performed on senior (age > 60) pre-travel consultation at the travel medicine clinic in Bordeaux, France. Among the travelers, 70 patients each year were randomly selected from 2013 to 2018. Results : Four hundred fourteen patients were included in this study. Their median age was 65 [IIQ : 62–68] with a M/F ratio of 1.03. A chronic medical condition was reported by 60.9 % (n=252) of the travelers and a chronic medication was taken by 62.8 % (n=260) of them. Yellow fever vaccine was administered to 43.7% (n=181) of the travelers. Typhoid fever vaccine was prescribed to 38.2 % (n=158) of the travelers and hepatitis A vaccine to 20.3 % (n=84) among them. A DTaP/IPV booster was prescribed to 68.9 % (n=84) of the consultants who were not up to date with vaccination. Malaria chemoprophylaxis was prescribed to 56.6% (n=230) of the travelers. The presence of a medical history (p=0.54) or a chronic medication (p=0.75) did not modify the indication for a prescription for antimalarial chemoprophylaxis. Conclusion : This study provides an overview of the senior pre-travel consultation across practices from a travel clinic.
Introduction : Le voyageur sénior présente des spécificités socio-médicales insuffisamment prises en compte lors de sa consultation pré-voyage (CPV). L’objectif de cette étude est de décrire les pratiques de vaccination et de prévention du paludisme lors de la CPV du sénior. Méthode : Nous avons réalisé une étude transversale observationnelle avec recueil des données rétrospectif de la CPV du sénior > 60 ans au centre de vaccinations internationales du CHU de Bordeaux. Un tirage au sort aléatoire a permis de sélectionner 70 voyageurs par année sur la période 2013–2018. Résultats : Un total de 414 voyageurs séniors ont été inclus dans l’étude. Leur âge médian était de 65 ans [IIQ : 62–68] avec un sexe-ratio H/F de 1,03. Une pathologie chronique était présente chez 60,9 % (n=252) des voyageurs et un traitement chronique était pris par 62,8 % (n=260) d’entre eux. Le vaccin antiamaril a été administré chez 43,7 % (n=181) des consultants. Les vaccins contre la fièvre typhoïde et contre l’hépatite A ont été prescrits chez respectivement 38,2 % (n=158) et 20,3 % (n=84) des voyageurs. Un rappel dTPca a été prescrit chez 68,9 % (n=84) des consultants non à jour de vaccination. Une chimioprophylaxie antipaludique a été prescrite chez 55,6 % (n=230) des voyageurs. Son indication n’était pas modifiée selon la présence d’un antécédent médical (p=0,54) ou la prise d’un traitement chronique (p=0,75). Conclusion : Cette étude apporte un éclairage sur la CPV du sénior au travers des pratiques d’un centre de vaccinations internationales.
