Attachement et troubles de l'attachement chez l'enfant placé. Évaluation clinique

Abstract : It is now accepted that the child placed in institution or in foster family can develop an attachment relationship with his new caregiver, under certain conditions. However, we can question its continuity and its long term stability. In addition, attachment disorders are defined by a significant difficulty in interactions with adults or peers. To date, in France, there is no recommendation by the French National Autority for Health (HAS) concerning the clinical assessment of attachment and attachment disorders in the child placed. However, the placement context, the use of various tools in studies and comorbidities, make diagnosis very difficult in clinical practice. The clinician has to gather information from several sources regarding the factors that influence the attachment system, the developmental history and medical history, the psychological functioning, and the history of attachment behaviors. Regarding clinical observation, he has to obtain a series of direct observations of the child interacting with his caregiver and with the unfamiliar adult. It is necessary to combine situations of play, learning and separation-reunion. Several tools for assessing attachment and attachment disorders are available, and can assist the examiner in clinical practice. The current state of scientific knowledge is not in favor of carrying out additional examinations, but additional assessments (psychological, psychomotor, speech therapy) need to be requested depending on the clinical manifestations.
