Le dépistage de la dénutrition de la personne âgée en médecine générale : sensibilité de l’outil SEFI

Abstract : Undernutrition is a prevalent health problem, which is still under diagnosed in France despite its potentially serious consequences, and the recommendations published and updated recently (2019). It’s considered that 4 to 10% of elderly people living at home are undernourished. Our case-control study, carried out from November 2019 to February 2020, aimed to analyze the sensitivity of the SEFI (Easy Ingesta Assessment Score) tool in general medicine, in people aged at least 75 years . This simple, fast and inexpensive tool is used today in hospitals or in general medicine in adults. Carried out on 117 patients consulted (64 controls and 53 cases), this work showed a sensitivity of SEFI for the diagnosis of undernutrition of 41.5% [confidence interval (CI) 95%: 28.4% - 55.8% ], a specificity of 93.7% [83.9% - 98%], a positive predictive value of 36.4% [19.3% -57.3%] and a negative predictive value of 94.8% [ 87.4% - 98.1%]. We were able to highlight that the SEFI tool could be used by general practitioners to eliminate the risk of undernutrition in people aged at least 75 years. It would thus save time during consultations, by selecting the people among the candidates for longer tests recognized by the HAS, such as the MNA, should be carried out to screen for this pathology. The negative predictive value is estimated to be more interesting in patients with a BMI greater than 25, where or wrongly, the risk of undernutrition is underestimated. SEFI is recognized by general practitioners as a quick and easy to use test. But on its own, it does not increase the screening rate for undernutrition in the elderly. General practitioners must be made aware of the gravity and consequences of undernutrition today. The small number of investigators requested for this study (13 participants) gives only a slight overview of the qualities of the SEFI tool in practical terms with regard to its use with elderly people. It would be necessary to conduct a study with a larger number of general practitioners giving their opinion on the design and handling of the tool to better adapt it to this type of population.
