Évaluation de la prise en charge de l’ostéoporose après fracture de hanche chez les patients de plus de 50 ans, dans le département du Lot-et-Garonne

Abstract : Introduction : osteoporotic hip fracture is frequent and complicated by morbidity and mortality excess. It is also associated with an increased risk of subsequent fractures and impose an heavy economic burden on our health care system. Patients with hip fractures should be assessed and treated as a priority in order. However, his medical care seems insufficient. The aim of this study is assess the prescription of anti-osteoporotic treatment, prescription of a bone densitometry, vitamin D and / or calcium supplementation, and emergence of a new fracture in the following year the hip fracture, in patients over 50 years old, in the department of Lot-et-Garonne. Methodology : it was an epidemiological study, retrospective cohort, multicenter. We identified all hip fractures in 2017 in the department of Lot-et-Garonne, and then random sample, stratified based on the activity of each establishment. Traumatic fractures or pathological fractures, patients residing outside Lot-et-Garonne or without family doctor were excluded. A questionnaire was sent to the family doctors of the patients included. Results : 668 hip fractures were counted in 2017 in the department of Lot-et-Garonne. The sample was taken from 250 patients ; 169 patients were included. Doctors response rate was 44%. The statistical analysis involved 75 patients. There were 56 (75%) women and 19 (25%) men with an average age of 83 years. One year after their fracture, only 12% had anti-osteoporotic treatment; 11% had a bone densitometry; 71% had vitamin D and / or calcium supplementation, and 24% had a new fragility fracture. Conclusion : the management of osteoporosis after hip fracture is still insufficient. The contribution of a fracture liaison services has been proved in many centers on French and international, and it would be interesting to make an similar epidemiological study after establishment of such a structure (which appears viable at the Agen hospital center).
