La médecine générale face au trouble bipolaire : le point de vue des patients

Abstract : Context: Bipolar disorder is a frequent pathology in general practice and is complex to identify . Few studies focus on the patient perspective. The aim of this study is to explore experiences and feelings of bipolar patients about the general practitioner care. Method: A qualitative study were realized, by conducting semi-structured interviews with diagnosed bipolar patients. Interviews were transcribed, then coded with a cross-analysis. Results: 20 interviews were conducted with diagnosed bipolar patients. 3 main topics were identified: the reason for consultation, global and psychiatric care. If the general practitioner is solicited in first recourse whether it is for a psychiatric or somatic reason, he’s hardly requested in quantity. Indeed, patients hardly consult their general practitioner. Regarding psychiatric care, communication between psychiatrists and general practitioners is often unsatisfactory and patients want more cooperation between these two specialists. They also want more trained clinicians in mental health and more support to accept the disease. However, patients seem really aware of the general practitioner limits in specialized care, yet capable of having a global perspective on the patient and a close relationship with him. Conclusion: This study explored patients experiences from the first sign of the disease to the actual follow-up. The role of the general practitioner in the disease seems unsatisfactory in most cases, and more training seems essential.
