Étude CalédoFaine : évaluation clinique du score de Faine chez les patients suspects de leptospirose au Centre Hospitalier de Nouvelle Calédonie, 2018-2019

Abstract : The diagnosis of leptospirosis is clinically difficult and the progression can be rapidly fatal in the absence of specific treatment. The Faine score, based on clinical and epidemiological criteria for its parts A and B, has been proposed by the WHO to assess the probability that a patient has leptospirosis. The CalédoFaine study assessed the relevance of the clinical and epidemiological criteria of the Faine score in patients suspected of leptospirosis in a case-control study conducted in 2018 and 2019 at the CHT in New Caledonia. All suspect patients admitted within the first 7 days of the disease were included and then classified as case or control according to the result of the leptospirosis blood PCR. 35 cases and 70 controls selected by drawing lots to respect the ratio of 2 controls per case were analysed. Multivariate analysis by logistic regression found a significant association between a "SF A+B" score taken from the categories "possible leptospirosis" (score 20-25) and "presumed leptospirosis" (score>26) and case or control status (p<0.0001). The performance of the model was high with an AUC of 99.27%, a sensitivity of 93.55% and a specificity of 96.36%, allowing the subjects to be correctly classified in 95.35% of cases. The CaledoFaine study validated the relevance of the Faine epidemioclinical score as a simple tool to identify possible cases of leptospirosis as soon as they enter the healthcare system. This score should be tested in other regions where differential diagnoses and lifestyles are different.
