Le patient âgé et ses médicaments au domicile : état des lieux de la connaissance et de l’adhésion thérapeutique par l’unité d’évaluation gériatrique pluriprofessionnelle dans la cité de Bordeaux

Abstract : With the expected population growth, improving the care of our seniors is a major public concern. Devices are set up by the health authorities, such as the national project Elderly People at risk for loss of autonomy, which aims to preserve the autonomy of people over 75. In this perspective, the multi- professional geriatric assessment unit in Bordeaux, established in 2015, visits seniors at home, at the request of general practitioners, in order to perform standardised geriatric assessment on patients with geriatric and/or social problems. The goals are to develop a multidisciplinary work, to facilitate ageing at home, to avoid emergency hospital admissions, to improve the elders’care, and to fight against iatrogeny. The aim of our study is firstly to make a thorough report about the patients’ drug knowledge and therapeutic adherence while visiting them at home and secondly, to assess the impact of definite factors on these two data. Our study was conducted over the year 2018 on all the patients treated by the EMG, using a questionnaire. Among the 167 patients assessed, we have estimated that one third of them know their treatment satisfactorily. The use of a pill organizer, the intervention of a nurse at home, and the high number of drugs have a negative influence on the patients’ drug knowledge, whereas it is improved by the lack of cognitive deficits or the preservation of independence for the supply and preparation of their drugs. As for the assessment of therapeutic adherence, only 37% seem to adhere well to their drug medication. The patients’ autonomy for the supply and the preparation of their drugs along with the lack of cognitive deficits improve drug adherence, whereas being a woman and polydrug use alter this parameter. Finally, according to our study, a good knowledge of the treatment is not correlated to a good therapeutic adherence.
