Évaluation de l’efficacité et de la tolérance des différentes antibiothérapies probabilistes per-opératoires utilisées dans les infections sur prothèses orthopédiques (hanche, genou et épaule) au CHU de Bordeaux

Abstract : Background: In Prosthetic joint infections (PJIs), some clinical situations require a per-operatory empirical Antimicrobial therapy (PEAT). The aim of this study was to evaluate the efficacy and tolerability of the various PEAT used at Bordeaux University Hospital. Patients and methods: The study included retrospectively all patients who received a PEAT for PJIs of hip (PTH), knee (PTK) and shoulder (PTS), in our center during the year 2018. PEAT details, adequacy of PEAT with the susceptibility of micro-organisms isolated, infectious relapse and adverses events (AEs) have been collected. Results: During study period, 54 patients were included (median years: 71 yo): 20 PTH, 30 PTK and 4 PTS. Per-operatory cultures were monomicrobial (54%), polymicrobial (17%) and negatives (27%). Seven PEAT were used : the association of piperacillin/tazobactam with linezolid was used in 77.8% of the patients. This association was active on 95.6% of the micro-organism founded : all the staphylococcus (n = 23) was susceptible to linezolid except one S. epidermidis. Three (5.5%) infectious relapses were observed. Twenty-two AEs were observed on 18 patients (42.8%) including 1 acute kidney injury (grade 3). Discussion-Conclusion : The association of piperacillin/tazobactam appears to be an effective and well-tolerated PEAT.
