Approche centrée sur le patient : le cas des troubles neurodéveloppementaux. Enquête auprès d’orthophonistes

Abstract : The position of the patient was upgraded during the 20th century, and with this evolution a more patient-centeredness approach developed. Studies underline the beneficial effects of this approach in the medical care of chronic diseases. This centered approach seems to be suitable for speech therapy care, however, there are few studies on the subject. The main objective of our study was to see whether and how liberal speech-language therapists apply this approach when caring for neurodevelopmental disorders. Our secondary objectives were the identification of influencing factors such as the practitioner's number of years of practice or his type of practice. We also wanted to check their knowledge of this centered apporach, and the existence of possible limits to its application. To do so, we conducted a survey by interviewing eight liberal speech and language therapists. Our results have shown that speech and language therapists apply this approach, but do not do it the same way. The speech therapist's type of approach (classic or evidence-based) would also be a factor favoring the power given to the patient. However, there would be certain limits that hinder the full use of the approach, which, according to the speech therapists interviewed, remains poorly conceptualized. Despite the small population studied, the study offers an interesting overview of the patientcenteredness in speech and language therapy. Larger studies, would be helpful to conceptualize the approach and make it suitable for the specificities of the profession. The centered approach is necessary to keep the human side of our care which tends to become more technical and scientific with the emergence of approaches like evidence-based-practice.
