Évaluation du dépistage et de la prise en charge de l’obésité infantile par les médecins généralistes en Guadeloupe

Abstract : INTRODUCTION : Childhood obesity has been a major public health issue for decades. The evolution is almost epidemic worldwide, especially in Guadeloupe. Diagnosis and medical care of obese children is one of the general practitioner’s roles. Our objective was to reveal the general practitioner’s difficulties to take diagnosis and take care of obese children in Guadeloupe and to suggest some solutions. MATERIAL AND METHODS : A 2 stage-study was realized with the general practitioners in Guadeloupe : a qualitative stage by semi-structures questionnaire, then a quantitative stage by phone and mail. RESULTS : Childhood obesity was defined as BMI > 2 standard deviations by 46%, 39% defined it as BMI > 30 kg/m2. Therapeutic patient education was proposed by ¾ of the practitioners. 82% of whom regularly scheduled consultations with those children, among them 31% added a specialized medical care by a specialist or in a clinic. If obesity persisted, 71% sent the child to a specialist. The two major obstacles to therapeutic patient education was the lack of parents/children’s motivation and the absence of specialized clinic. 69% think it could be remedied by the creation of new childhood obesity clinics, 50% think it could be remedied in raising awareness among the population about the risks of obesity. The opening of an obesity specialized hospital is awaited by 90% of the doctors. CONCLUSION : The general practitioner role in childhood obesity diagnosis and medical care is very important, most of the doctors realized it. There is a lot of obstacles, making medical care very complexed. The project of childhood obesity hospital could meet the general practitioners’ expectations. However, general practitioners remain important for those young patients and their family’s medical care. Improving information is necessary in order to maximize prevention and diagnosis of overweight and obesity.
