Perception de la prise en charge des enfants présentant un trouble du neurodéveloppement par une équipe de soins pluridisciplinaire

Abstract : Context: Children with a neurodevelopmental disorder represent a complex population in ambulatory medicine. Their healthcares needs can’t be reduced to a single area and require a multidisciplinary coordination of cares. Objective of the study: The objective of this study is to identify the problems and expectations of a multiprofessional team from a health centre concerning the care of children with a neurodevelopmental disorder, in order to create a healthcare protocole. Methodology: This is a qualitative study using semi-structured interviews. Twelve interviews were carried out between February and March 2020. The sample included professionals from the multidisciplinary team of the Cruseilles health center (general practitioners, speech-language pathologists, ergotherapists, psychomotrician and orthoptist). A thematic analysis with empirical orientation was made. Results: Five areas of difficulty were identified. All stages of the healthcare pathway were concerned, from the establishment of the diagnosis to the communication between the participants, including care’s orientation and coordination. Meanwhile, access to care was limited by a saturated network and expensive long-term rehabilitation. One of the limitations found was the lack of training for medical workers. The teachers’ lack of knowledge and the insufficiency of communication between the school and health professionals had a negative impact on the identification and implementation of school facilities. Moments of discussion with the family as well as support should be included in the child's process. However, the main element highlighted by this study was the desire of professionals to have tools available to facilitate the orientation of children by prioritizing care according to clinic. Conclusion: The problems and expectations raised by this work will make it possible to achieve a care protocol.
