Exploration des facteurs de risques suicidaires dans une population d'adolescents pris en charge en service de pédopsychiatrie dans le Var

Abstract : Introduction : In 2014, in France, there was an average of one suicide every hour. Psychiatric disorders can be found in up to 90% of teenagers who committed suicide. Therefore, it might be hypothesized that a suicide attempt and especially repeated suicide attempts could partly be explained by an undiagnosed mental illness in adolescence. The main objective of the study is to describe comorbid psychiatric disorders of suicidal behaviour in accordance with the DSM-5 disruptive behavior criteria. The secondary objective is to enhance suicide prevention.
Methods : We propose an observational cohort study over a 6 months period. This study included adolescents from 11 to 18 years old admitted for attempted suicide or for having active suicidal ideation at the CMPEA in Hyères or at the Villa Nova. The exclusion criteria was the impossibility to administer the K-SADS-PL by interviewing the parent(s) and/or the child involved. Socio-demographic data, and personal and family history have been collected to assess patients’ psychological condition. Secondly, the Mini-kiddie, the K-SADS-PL, the RUD and the MADRS have been conducted. The patients were distributed into three groups : « No suicide attemps », « suicide attemps » and « repeat suicide attemps ». The primary endpoint of the study is the recidivism rate.Results. From November 2018 to November 2019, we included 17 patients. Three of them were lost to follow up. We performed this study with 14 teenagers, exclusively girls, aged between 13 and 17 years. The K-SADS-PL showed that mood disorders were present : 28.6% had major depressive episode; 28.6% had dysthymic disorder; and 14.3% had bipolar disorder. The prevalence of schizoaffective disorder was also significant with 21.4% of cases.
Discussion : In our study, a psychiatric disorder was diagnosed in 92.8% of cases. Repeated suicide attempts were strongly represented with a rate of 35.7%. The mean time between two suicide attempts was approximately four and a half months.The small proportion of patients does not allow us to provide data regarding the epidemiology and the etiologies of the suicidal adolescents. Data were fairly representative of the international literature. Improvement of the diagnostic procedures is essential, in particular through the use of standardized assessments and appropriate treatment. The development of new technologies is of particular interest
