Vers une meilleure qualité des soins pour les bénéficiaires de l’Aide Médicale de l’État (AME) en Gironde : enquête qualitative par entretiens individuels auprès des médecins généralistes des bénéficiaires adultes de l'AME de Bordeaux Métropole

Abstract : Introduction: every year, interactions between human society and environment push millions of people on the road of exile. Many of them arrive in France passing through exhausting migratory process. There, walks of life and backgrounds damage their health and the french measure called AME (Aide Médicale de l’État) is one of the recourses helping them to access medical care. The main goal of this study is to explore knowledges, representations and treatment strategies of Bordeaux general praticioners with AME and its adult recipients in order to improve primary health cares toward this specific population. Method: qualitative research with semi-directive interviews achieved with twenty general pratitioners practicing in Bordeaux. Outcomes: interviewes’ knowledges about AME system and its beneficiaries was restricted. All of them approved it ; for some of them it was measures to develop, for others a system to restrict. Two types of physicians were highlighted : the ones who consider AME beneficiaries similar to other patients with particularities and the ones who understand the impact of this characteristics without knowing exactly how to efficiently overcome. The main problem encountered is the difficulty to communicate simply and with confidence. Follow-up cares, screenings and preventions, specialized cares and psycho-social involvement were also more difficult. Conclusion: in order to improve this persons health, it is necessary to involve professional interpreters. Adaptability of individual centered pratices, health mediators’ role developpement, network cares includeed specialists and surgeons, medico-social organisations or every professionnel liable to enhancement of life contexts are also encouraging leads. Awareness to healthcare and precarity are essential, from the further studies to regular trainings, to promote equity in health.
