Obésité sarcopénique : étude descriptive rétrospective d’une population gériatrique hospitalisée

Abstract : Introduction: muscle aging and the increased prevalence of obesity in the geriatric population create a new area of research: sarcopenic obesity. In prospective cohorts of non-hospitalized subjects, it is associated with an increased risk of developing physical limitation. Hospitalization is well known event at risk of loss of independence. The impact of sarcopenic obesity during this episode isn’t known yet. Objective: analyze the evolution of autonomy during a hospitalization in an acute geriatric ward, looking for a link between the presence of sarcopenic obesity and an increased loss of autonomy. Early readmission, length of stay and changes in body composition during hospitalization were also examined. Materials and Methods: retrospective descriptive monocentric cohort study carried out in an acute geriatric ward of the Pau hospital. Sarcopenia is diagnosed using the European Working Group on Sarcopenia in Older People algorithm by an impedancemeter. A BMI over 30 is used to report obesity. Autonomy is rated on the Katz scale. Results: 92 patients were included. Sarcopenic obesity was diagnosed in 5.43% of cases, sarcopenia and obesity in 85% and 21% of patients, respectively. The greatest variation in autonomy during hospitalization was observed in sarcopenic obese patients (mean variation of 2 out of 6 points, p=0.046). A total of 15 early readmission at 1 month were counted, with the highest rate for sarcopenic obese (60%, but 16% at the sample level) (p=0.045). The average length of stay is 17.4 days. On average, weight loss during hospitalization was 1.6kg, but in obese patients, it is greater (3kg). Conclusion: sarcopenia is common in patients hospitalized in geriatrics, and when associated with obesity, there is greater variation in autonomy and readmission.
