Évaluation de la qualité de vie et du fardeau des aidants des patients en post AVC dans la population gériatrique

Abstract : Introduction: 150 000 people suffer from a stroke each year in France, 75% of which are 65-year-old or older. Moreover, 40% of patients suffering from stroke have after-effects which makes stroke the first cause of disability in France. In this context, long-term morbi-mortality of major stroke victim caregivers increases. Out study aims to study the early consequences on the quality of life and the burden of the primary caregiver of geriatric patient who had a stroke. Material and methods: from March 2018 to January 2019, 39 major caregivers have been evaluated during their relative hospitalization on the Geriatric Follow up Care and Rehabilitation Unit of Xavier Arnozan Hospital of Pessac (France) then 3 months after their hospital check out using two validated questionnaires: ZARIT scale and SF-3-. Results: the burden of the primary caregiver assessed by the ZARIT scale is significantly higher à M=3 months than it is as M=0 during the hospitalization (mean: 25.8 at M0 vs 33.3 à M3; p<0.0001). About SF-36 dimensions quatifying quality of life there is a significant alteration between M=0 and M=3 months of major caregivers limitations dues to physical condition (60.9 vs 50.6; p=0.034), vitality (47.9 vs 42.7; p=0.010), social relationships (73.3 vs 65; p=0.011) , pain (69.8 vs 60.3; p=0.0004) and global health (60.4 vs 52.4; p=0.0005). Conclusion: an alteration in the quality of life and an increase in the burden on the primary caregiver of a patient over the age of 75 can be observed as early as 3 months after the patient's discharge from hospital. It could be interesting to assess major caregiver's quality of life and burden with simple means, during a dedicated general practice consultation or not, in order to adapt medical and paramedical aids to execute, and offer tainingto caregivers in order to reduce the morbidity and mortality inherent in their role as primary caregiver.
