Élaboration et validation d’un outil de promotion de l’activité physique par le médecin généraliste

Abstract : Lack of physical activity is responsible for 3.2 million annual deaths around the world and partly explains the increase in cases of chronic pathologies. Promoting physical activity is therefore essential. General practitioners, who are actors in primary care and preventive medicine, play a major role in this or effort.
The objective of this thesis is to develop a practical tool to promote physical activity in general medicine named OPAP. The Delphi method chosen to validate and improve the tool enables the development of a consensus through an iterative process in which experts are solicited via questionnaire. The tool was developed with the aid of a preliminary bibliographical study, accessible at this website http://promouvoirlactivitephysique.fr/. The principle of the tool is as follows: the doctor asks the patient four questions. Based on the responses, the patient is classified as "sufficiently active," "insufficiently active motivated," or "insufficiently active unmotivated."
The doctor then refers to the corresponding “doctor-patient” sheet. This sheet is divided into two sections: an “in consultation” section made up of questions/comments intended for the doctor as a consultation aid; and a “take home” section containing exercises and information for the patient after the consultation.
At the end of the study, the tool was partially validated using stringent statistical criteria. Its joint construction with the main actors, general practitioners, makes it relevant. The goal was to meet a real need for promoting exercise rather than achieve statistical perfection. Further evaluation of the tool’s use in outpatient medicine will allow additional improvements based on practical experience.
