Bilan médical initial en soins primaires des migrants arrivant sur le territoire français : revue systématique de la littérature

Abstract : Background: the health of migrants continues to be a challenge to both public health and the dignity of the host country as the migration phenomenon grows. Objective: the aim of this work is to gather data to form the basis of a common initial medical check-up protocol in the primary care of migrants arriving on French territory. Study design: a systematic review of the literature, including articles published between January 2009 and June 2019, was conducted according to the PRISMA recommendations. The databases PubMed, Cismes, LISSA, EM Premium, Scopus, Web of science, Cochrane, Pascal and Françis, Public Health Database were explored using designed search procedures. Each selection step was carried out independently by two researchers. Evaluation grids were used to assess the quality of each article. Of the 2134 articles examined, 70 were included in the final descriptive analysis. Results: the methods used to carry out the initial medical check-ups are heterogeneous among the various international recommendations. The main interventions in France are : questioning adapted to the patient's migratory path, a complete and directed clinical examination, including a search for physical signs of violence, a systematic biological check-up (CBC, ionogram, urea, creatinine, ASAT, ALAT) ; identification of mental health disorders and a link to follow-up procedures; an assessment of infectious, chronic and women's health pathologies with adapted contraception, and an updating of vaccinations. Conclusion: the modalities of this initial assessment should be the subject of official recommendations with a view to standardising practices and improving the conditions of arrival of migrant patients. Future work could focus on the cost of such a health check-up, its application in practice in primary care centres and paediatric care.
