Prise en charge d’une épidémie d’entérocoques résistants aux glycopeptides

Abstract : The spread of multi-resistant bacteria (BMR) is a major public health issue in France, Europe and the world. Among these multidrug-resistant bacteriae, the emerging Highly-Resistant Bacteria (eHRB) are among the most surveilled bacteria today. So as their role in nosocomial opportunistic infections has been increasing in recent years, several hospital epidemics involving multiresistant enterococci to antibiotics have been described.In this context, all health establishments can be affected by the emergence of this kind of epidemics, hence the importance of being vigilant and acting quickly as soon as a first case appears for the good control of the diffusion of emerging Highly-Resistant Bacteria (eHRB) such as enterococci resistant to glycopeptides (ERG) or enterobacteria producing carbapenemase (EPC). The aim of this study is to establish an inventory of the current worldwide emergence of enterococci resistant to glycopeptides and to assess the measures implemented during an enterococci resistant to glycopeptides epidemic at the Hospital Center Périgueux, France, which happened from March 2018 to June 2019. The microbiology laboratory of the Hospital Center Périgueux detects on average 3-4 strains of Enterobacteriaceae producing carbapenemase OXA 48 per year, so the appearance of this epidemic was a great challenge for the laboratory. The pillar of the strategy adopted at the Hospital Center Périgueux for controlling the diffusion of enterococci resistant to glycopeptides was the guide developed by the High Council for Public Health in 2013, which updates and harmonizes all the existing recommendations in France concerning the prevention and transmission of emerging Highly-Resistant Bacteria (eHRB).
