Repérage à la naissance des nouveau-nés à risque de développer un surpoids ou une obésité infantile. Quels facteurs anté et post nataux sont les plus pertinents et potentiellement prometteurs dans l’évaluation de ce risque ?

Abstract : During the perinatal period called as the "first 1000 days", several maternal and obstetric risk factors are known to be linked with overweight and childhood obesity. Our work was a bibliographic study of these risk factors based on 160 articles including a lot of meta-analysis followed by an epidemiological study collected from a database of women who gave birth in the maternity hospital of Bordeaux University Hospital between 06/01/2016 and 06/30/2017. The project aim was to suggest a screening tool for newborn at risk who could develop childhood obesity and which can be used from birth. On the 1977 mother-child couples involved, we defined the prevalence of the following risk factors: obesity and mother overweight at the beginning of pregnancy, excessive weight gained during pregnancy, gestational diabetes, smoking, precarity, method of delivery, newborn weight, hypotrophy and macrosomia ; and we found high prevalences of these risk factors. Then we compared the characteristics of women and their newborns according to the different risk factors. From the analysis of studies and those results, we proposed a draft of risk score containing major and minor criteria concerning the mother and the newborns. This draft would let us identify from birth, newborns with risk of developing overweight or obesity during their childhood. In the first place, these work outlooks are to confirm the interest of this score in the identification of newborns at risk from a children cohort. Secondly, this score can be used as a basis to detect newborns at risk for intervention studies to prevent childhood obesity planned by the Specialized Pediatric Obesity Center of the Bordeaux University Hospital.
