CRYBB : que cachent les pleurs du nourrisson au service d’accueil des urgences pédiatriques : étude descriptive au CHU de Grenoble

Abstract : Objectives: To determine the diagnosis retained during consultations for infant crying at the pediatric emergency department. Our secondary objectives were to analyze the factors associated with pathological crying and the description of their management. Methods: In this unicentric study, under five months infants consulting for crying were included from November 2019 to June 2020. Fever was an exclusion criterion. Perinatal data, nutrition data, crying characteristics, physical examination, diagnostic test and the infant’s outcomes were collected. The retained diagnosis was classified as pathological or non-pathological crying. Results: A total of 89 infants < 5 months (5,5%) had consult for crying. A quarter had pathological crying (n = 22, 25%). The main etiology was infectious (n = 9, 41%). Age> 6 weeks, no history of colic or GERD, presence of nutritional disturbances and / or crying characterized as unusual by parents were significantly associated with pathological crying (p≤0.05). The hospitalization rate was 21% (n = 19). The reconsultation rate was 40% (n = 36) which allowed 7 children (28%) to be re-qualified for pathological crying. Conclusion: Medical care is necessary for a quarter of infants consulting for crying. The diagnoses retained are varied. This study identified anamnestic factors that could be predictive of pathological crying. Further studies are needed to better target infants at risk at the first consultation.
