Contraintes biomécaniques et organisationnelles professionnelles chez la femme enceinte : définition des niveaux d’expositions par une méthode de consensus

Abstract : Context: In the workplace, the levels of exposure to biomechanical and organizational constraints leading to an increased risk of pathologic pregnancy are poorly known. This lack of knowledge leads to significant variability in occupational health practices. Objectives: To identify the biomechanical and organizational constraints influencing pregnancy outcomes and to define the exposure levels at which the risks for pregnancy are significant using a consensus method (Delphi). Methods: Carrying out a consensus method including a literature review and expert opinion on exposure levels. The variables studied are 1) exposures: night work / shift work, weekly working hours, heavy loads lifting, prolonged standing, multiple expositions; 2) pregnancy outcomes: prematurity, low birth weight, spontaneous miscarriages. Results: Weekly working time must not exceed 40 h/week; in the absence of consensus on the level of exposure, night and/or shift work must be avoided; prolonged standing must not exceed a 3 h/d; lifting must be limited to carrying loads < 11kg and with a daily load < 100kg; multiple exposures must be avoided, in particular: vibration, night work / shift work, working time exceeding 40 h/week, prolonged standing and heavy load lifting. Conclusion: these results should be used by the occupational physician to answer the question of whether an exposed employee should remain at work, while taking into account her individual characteristics (medical history, family situation, socio-economic level, etc.) and in consultation with pregnancy specialists (obstetricians, midwives).
