Expression de l’enzyme immunosuppressive IL4I1 dans les lymphomes B et T cutanés primitifs : un rôle dans l’échappement tumoral ?

Abstract : Immunosuppressive enzymes are involved in the immune escape of cancers, and can be therapeutic targets. Their expression have never been studied in cutaneous lymphoma (CL). In a retrospective, single-center and descriptive study, we analysed using immunohistochemistry the expression of the IL4I1 immunosuppressive enzyme in a series of 67 B and T-cell, indolent and aggressive CL. IL4I1 was expressed in all cases by M2 macrophages (CD163+) of the tumor micro-environment (TME). A higher proportion of IL4I1+ cells was found in aggressive CL (p=0.38), with significative differences in transformed mycosis fungoides (TMF) and Sézary versus early MF (p=0.018), and in anaplastic CL versus lymphomatoid papulosis (p=0.0012). In these two models of CL, characterization of the TEM showed an increase in CD8+, granzyme B+ effector T lymphocytes (TL), and a reduced proportion of FoxP3+ regulatory TL. These results, contrasting with those obtained in solid cancers, suggest that TEM and immunoregulatory effectors have a more complex function in CL, where neoplastic cells are immune cells by themselves, thus potentially targeted by immunomodulators. Immunotherapy should thus be used with caution in this context. This is the first study focusing on an immunosuppressive enzyme in LC. The involvement of IL4I1 in promoting the aggressiveness of LC, as well as the potential role of other immunosuppressive enzymes (iNOS, IDO, Arginase), remains to demonstrate by studying larger series of cases and through functional approaches.
