Déclenchement artificiel du travail et utérus uni cicatriciel : facteurs pronostics de la réussite de la tentative de voie basse après césarienne Cohorte rétrospective de femmes présentant un utérus uni-cicatriciel et prises en charge pour un déclenchement artificiel du travail dans une maternité de niveau III

Abstract : Introduction: The uterine scar is a risk factor for uterine rupture (0.2 to 0.8%), this risk is doubled when it is an artificial induction of labor. But the success rate of a trial of labor after cesarean delivery remains high (60%) in a context of induction of labor. Many prognosis factors are found in the literature concerning the trial of labor after cesarean section (TOLAC) with spontaneous labor but a few when it is an artificial induction of labor. Objective: The objective was to identify the factors of good prognosis, of TOLAC in the context of an artificial induction of labor. Methods: Retrospective study about a cohort of 62 patients with an uterine scar in the context of an artificial induction of labor. Setting: North Hospital in Marseille, level III, between january 2015 end june 2018. Results: The study founds 61.3% of patientswho had a vaginal delivery, 38.7% had a cesarean. The study found also that the absence of diabetes as well as the absence of an abnormal fetal heart rate are factors associated with the success of TOLAC. Moreover, when the gestational age and the BMI at birth are increased, this decreases the chance of success of vaginal birth. Conclusion: Knowledge of prognosis factors for the success of TOLAC would help guide the medical team in the care and the monitoring of these patients and provide more precise information to women regarding the chances of success of TOLAC in the context of an induction of labor.
