Morbidité néonatale après retour à domicile sur une cohorte de nouveau-nés pris en charge dans le cadre de l’accompagnement global en maison de naissance : étude descriptive sur 36 mois

Abstract : Objectives – Quality assessment of postnatal care in midwifery units (MU) of healthy term newborns with early discharge before 24h of life (ED<24h) : measure the frequency of newborn readmission (ReAdm) to hospital from ED<24h to 28th day of life (D28), assess good professional practices for discharge eligibility and post-discharge follow-up. Materials and methods – Longitudinal study of a newborn cohort over a period of three years following the beginning of operations of the MU in Grenoble. Main outcome measure : ReAdm from ED<24h to D28, corresponding with unplanned consultation and/or hospitalisation. Results – 203 newborn with ED<24h : 1/3 from primiparous women, 91% were breastfed. 10 newborn were readmitted, of which 8 males (p=0.049). ReAdm rate = 4.9% ; appearance rate for ReAdm = 67 new cases/100 newborn/year (incidence = 0.67 IC95[0.26 ; 1,08]). 80 % of ReAdm occured before D8 of life, caused by jaundice and infection ; 80 % of readmitted newborn had a length of stay at hospital <48h. 100 % of readmitted newborn were eligible for ED<24h. We found 100 % of conformity for post-discharge follow-up. Conclusion – Healthcare coordination in MU in Grenoble provides the safety of newborn after early discharge.
