Évaluation de la précision du système KneeAlign 2® dans le rétablissement de l’axe mécanique neutre du membre inférieur lors de la réalisation d’une prothèse totale de genou

Abstract : Introduction: The KneeAlign 2® system is a portable accelerometer device for the navigation of total knee arthroplasty (TKA). It aims to combine the precision of computer-assisted surgery (CAS) with the ease of conventional cutting guides. Hypothesis: Our hypothesis was that the use of the KneeAlign 2® system would limit to less than 10% the knees presenting a non-optimal alignment and / or positioning of the femoral and tibial implants. Materials and methods: Our study was retrospective, single-center. 52 knees (42 genu-varum, 10 genu-valgum) underwent total knee arthroplasty with the KneeAlign 2® system. The postoperative global mechanical alignment (HKA) as well as the alignment of the femoral (alpha) and tibial (beta) implants were measured on radiographs of the major frontal axes of the lower limbs. The non-optimal values were defined by a difference of more than 3 ° for the HKA angle and a difference of more than 2 ° for the angles alpha and beta from the neutral mechanical axis. Results: Postoperatively, 23.07% of the knees presented a non-optimal mechanical alignment of the lower limb, 13.46% of the knees presented a non-optimal positioning of the femoral implant and 23.07% of the knees presented a non-optimal positioning of the femoral implant. tibial implant. Discussion: Our study showed that the KneeAlign 2® System provided optimal alignment in 76.92% of procedures. Comparison to data in the literature seems to show that the accuracy of the KneeAlign 2® system does not reach the precision of CAS, but approaches conventional cutting guides. However, it seems to reduce the risk of unacceptable positioning of the implants compared to conventional surgery.
