Le dosage plasmatique des antipsychotiques

Abstract : Therapeutic drug monitoring consists of measuring and adjusting the plasma concentration of an antipsychotic neuroleptic in order to optimize drug management. Schizophrenia is a chronic, invalid pathology that can be managed over the long term. Currently, the objective of drug treatment for this pathology is to deliver a dosage for which the saturation of dopaminergic D2 receptors would be between 60 and 80%, thus defining the therapeutic target. Below this interval, the clinical response would be partial or zero, and beyond that there would be a risk of occurence of invalid side effects. But the treatment is metabolized by the body, before it reaches these neural receptors; it is therefore necessary to take a closer look at the plasma concentration of the drug for which therapeutic target is reached. The metabolism of patients presents a great interpersonal variability justifying taking into consideration the many influencing factors. They can be specific to the patient, because of his genetic peculiarity generate an accelerate or on the contrary slow cytochrome activity, but also of his age, his gender or his body mass. Also, they can be external to the patient, because of his lifestyle, his consumption of tobacco for example or even of co-prescribed treatments. All these factors are progressive and can be modified throughout the life of the patient and therefore of his care. The determination of the plasma antipsychotic levels would facilitate close monitoring by allowing the acute phase of the disease to judge the effectiveness of a treatment and to guide the therapeutic decision quickly and in the long run to define the metabolic profiles of the patients, anticipate potential variations in plasma concentrations and thus optimize the dosage of treatment.6 This would lead to improving the well-being of patients, but also to reducing the risk of relapse and therefore preventing possible hospitalizations, which would constitute a benefit in terms of cost of care.
