Évaluation des délais de prise en charge des accidents vasculaires cérébraux bénéficiant d’une thrombolyse intraveineuse aux urgences de Saint-Pierre du CHU de la Réunion

Abstract : Background : Thrombolysis must be done as early as possible in the treatment of ischemic strokes to allow a a better efficiency. International recommandations advocate a delay between emergency room arrival and thrombolysis , door-to-needle time, of less than 60 minutes for more than 50% of the patients. The primary outcome is the median door-to-needle time of the patients who benefited from thrombolysis in the context of ischemic stroke in the emergency department of Saint-Pierre in the University Hospital of la Reunion in 2019. Method : This was a single-center observational study. We collected the data of all patients who presented to the emergency room within a timeframe compatible with a thrombolysis in 2019. Among these patients, we analysed the door-to-needle time of those who received thrombolysis. Patients who showed signs after arriving at the hospital were excluded from the analysis. Results : Among the 417 patients with suspected stroke, 62 patients had thrombolysis in 2019. The median door-to-needle time was 65 minutes. 25 patients had thrombolysis in 60 minutes or less and 37 in more than 60 minutes. The median delay was 180 minutes from onset to thrombolysis and 30 minutes from arrival to imaging. Conclusion : Some efforts remain to be made in order to decrease door-to-needle time. Cooperation between the various stakeholders in the care of these patients must be optimized to speed up the patient circuit. Various measures remain applicable in order to reduce the time taken for care.
