Évaluation des pratiques d'échographie clinique réalisée par le médecin urgentiste (ECMU) dans la prise en charge des arrêts cardia-respiratoires (ACR) en préhospitalier au SMUR 64B de Pau

Abstract : Context: the place of ultrasonography made by emergency physicians in the management of out-of-hospital cardiac arrest (OHCA) isn’t fully defined yet. Only a few protocols are trying to secure the way of using it, but none of them had been studied in a French emergency medical service (EMS) whom medicalize those interventions. OBJECTIVES: We studied the use of ultrasonography made by emergency physicians during the management of cardiopulmonary resuscitation (CPR) in the French prehospital setting. The main objective of this study was to assess the number of physicians using the ultrasonography during CPR of OHCA, and then, to study the way of using it, their knowledge and expectations for a new protocol. Methods: through a survey submitted during individual interviews, we analyzed the physicians’ practice of EMS in Pau, France. Results: clinical ultrasonography was used by 23 of the 33 physicians of Pau’s EMS. However, only 5 physicians knew one of the protocols described in medical reviews and only 4 out of 33 had applied one of them. 30 of the physicians was expecting a service’s protocol, and 26 felt the need of a special training. Conclusion: ultrasonography in the management of OHCA arrest is commonly used in our population and a large majority of the physicians is expecting a protocol to standardize their practice. In the end of our study we propose a protocol answering expectations of the physicians of Pau’s EMS.
