Lupus érythémateux systémique et anticorps anti-ADN natif : comparaison de méthodes entre un ELISA « maison » et des techniques automatisées

Abstract : Although anti-double stranded DNA IgG assays are calibrated using an international standard, the polyclonal immune response, the diversity of auto-antibodies produced, and the intrinsic differences between assays, make comparisons difficult between different tests. Different groups of lupus patients were selected and analyzed in order to take into account clinical heterogeneity and the different stages of the disease. We compared a "in-house" ELISA, with three automated kits: the CLIA from IDS on iSYS, the CLIA Quanta-Flash from Werfen on Bioflash and the Elia from Thermofischer on Phadia (now Unicap). The ROC curves were determined using sera from patients with lupus at the time of diagnosis, free from any treatment and we have adapted the cutoffs to improve performances and to overcome values of undetermined significance, provided by the suppliers. Sensitivity/specificitiy with these cutoffs were 89.3%/100% for the “in-house” ELISA, 89.3%/97.1% for the Bioflash, 75%/100% for the iSYS and 82.1%/85.3% for Phadia. Analytical and clinical concordance criteria were defined in order to compare those tests. Bioflash and iSYS were less discordant with the “in-house” ELISA (8.6% and 9.3%, respectively), than Phadia 19.8%. The expected benefits, in terms of ease of use and traceability, without significant deterioration in performances compared to the "in-house" ELISA, allow automation of this dosage using CLIA technique.
