Association entre l’administration d’oxytocine en perpartum et les difficultés de mise en place de l’allaitement maternel.
Une étude de cohorte rétrospective unicentrique.

Abstract : Introduction: suboptimal breastfeeding is a national public health problem. In fact, in France, there is a frequency of breastfeeding and a duration of execution lower than the recommendations of the World Health organization. This reality justifies the studies seeking the determinants of these breastfeeding defects. Among these determinants, the peripartum data are still not really studied despite of their importance. It seems particularly important to us to mention one in particular: the administration of oxytocin in view of the central place of oxytocin in the hormonal scheme of achieving breastfeeding. Materials and methods: we carried out a quantitative observational study exposed / unexposed retrospective unicentric. We compared the difficulties of initiating breastfeeding in a group of women not exposed to oxytocin (n = 99) and exposed to oxytocin during labor (n=95), during postpartum immediate(n=65) and during labor and postpartum immediate (n=59). Results: the difficulties of implementing breastfeeding in the population not exposed to oxytocin concerned 39% of the patients; these difficulties appear to be increased in all groups exposed to oxytocin perpartum. In fact, patients exposed to oxytocin during labor presented difficulties in setting up breastfeeding in 64.8% of cases (p <0.001) and in 57.6% of cases in patients exposed to oxytocin during labor and during the immediate postpartum period (p = 0.013). Concerning the patients receiving oxytocin during the immediate postpartum only, they were 50.8% to have difficulties but this result was not significant (p = 0.067). However, the association between oxytocin and breastfeeding difficulties was not found after adjusting for confounding factors in multivariate analyzes with an Odds Ratio of 1.44 [0.6-3.49] for administration during labor and 3.01 [0.96-9.48] for administration during labor and postpartum. Conclusion: it seems that there is no association between oxytocin administration and breastfeeding difficulties. However, it is likely that the course of labor, delivery and birth has a real impact on the initiation of breastfeeding, which deserves to be studied in the future.
