Association entre lésion cérébrale et apnées du sommeil après un infarctus cérébral : étude transversale

Abstract : In this cross-sectional study, we aimed to investigate the potential link between lesion location and SAS at a sub-acute delay post-stroke. 142 patients, aged between 18 and 85 (median=60 [50.75; 69], females 29%), were included in the study after a first ischemic stroke. Patients with transient ischemic attack, haemorrhagic stroke or treated SAS were not included. Patients underwent a polysomnography between 3 at 6 months delay after stroke. The median of National Institutes of Health Stroke Scale (NIHSS) was 3 [1; 6.25]. Stroke location was classified on the basis of acute brain imagery (brain magnetic resonance imaging or computed tomography scans). Lesions were described and classified according to their anatomic and vascular territories. Moderate to severe SAS (apnea-hypopnea (IAH) >=15 events/hour) was found in 46% patients (52% obstructive SAS, 25% coexistent SAS and 22% central SAS). SAS patients had more infra-tentorial lesions than patients with no SAS. Specifically, lesions of the cerebellum were significantly more prevalent in SAS patients (p=0.003). Consistently with the aforementioned association, ischemic stroke due to posterior inferior cerebellar artery (PICA) occlusion was more frequent in SAS patients (p=0.01). These results are consistent with the common belief that acute strokes often cause sleep apnea, and open perspectives in the identification of a neuroanatomical substratum to SAS.
