Évaluation d’un changement de techniques de laboratoire pour la recherche de mycobactéries non tuberculeuses chez des patients atteints de mucoviscidose

Abstract : Decontamination of sputum from patients with cystic fibrosis for the detection of non-tubercular mycobacteria (NTM) is essential to limit the risk of contamination of the culture media by the bacterial flora. This decontamination in CHU takes place in two stages: NaOH NALC then addition of oxalic acid. The objective of this study is to compare contamination rate and viability of NTM by a simple decontamination with NaoH-NALC (D1) or double decontamination (D2). This study took place at the CAEN University Hospital from 27/12/19 to 21/07/20 with the inclusion of 42 patients, i.e. 76 sputum. 4 patients tested positive for NTM (9 samples), 2 for MABSC (already known), 1 for M. lentiflavium, 1 for a contaminating NTM : M. gordonae. A difference in the growth time on positive patients was observed on a 4/9 samples with a 10 to 14 days faster growth time in favour of D1 (the average growth time on D1 is 14.4 days compared to 27.8 days for D2) and an exclusive push on the D1 for 2 samples from the same patient. The contamination rate of the media is higher on D1: 29% versus 9% on D2. This study shows that oxalic acid does have an impact on the viability of NTM iand it may lead to an underestimation of the number of NTM identified in patients with cystic fibrosis. It seems to us then essential to change seeding techniques. At present, all sputum will be decontaminated by protocol D1. Contaminated samples will be treated by the D2 method, in a second step if necessary.
