Évaluation de l’utilisation de la mesure du glucose interstitiel sur l’équilibre glycémique de la cohorte pédiatrique de patients diabétiques suivie dans l’unité d’endocrinopédiatrie du CHU Sud Réunion

Abstract : Background : The challenge in the management of paediatric diabetes is to maintain an acceptable quality of life and to prevent any complications. This factor is in correlation with the number of blood glucose monitors. New technologies can be now propose to measure continuous glucose by wearing an interstitial glucose sensor (IGS) and they are more and more used in paediatrics. The use of these devices need to be evaluate. Methods : Our observational monocentric study has included 81 insulin-requiring diabetic children followed at the hospital of South Reunion, either current or former IGS carriers. The HbA1c 6 months before and 6 months after putting of the IGS and their annual averages has been compared. An assessment of the use, tolerance and level of therapeutic education (TE) has been carried out by a questionnaire. Results: IGS did not improve glycemic control: the HbA1c dicreased from 8.47% to 8.42% in the space of 6 months after the putting, and the annual average went down from 9.11% to 8.33% (p value 0.85). Few patients have reached TE level 3, so the use of the sensor is not optimal. One of the major points that emerge from our study is the improvement in quality of life thanks to the wearing of these sensors. 44% of the children have showed a skin reaction to IGS, which appears as an obstacle to the use of the sensor. Conclusion : These IGS bring an improvement in the quality of life of patients even if they do not improve glycemic control. We notice a skin intolerance, which constitutes a barrier to the use of the sensors. Improving management also requires better levels of therapeutic education.
