Prévention et dépistage de maladies infectieuses chez les populations migrantes, mission de la PASS : réflexion éthique sur l'autonomie et le consentement des patients

Abstract : From the outset, France has welcomed immigrants people from all over the world. Specialized organizations take charge medically and socially of these populations, including PASS: Permanent Access to Health Care, starting point of this work. One of the public health's mission of PASS is to carry out screening and care of immigrants from endemic countries for infectious diseases, transmisible between humans. But, by the particularity of these patients; langage, cultural ans administrative barrier; a lot of questions have come up from this mission.
Material and method: a job shadow was made in the PASS of the Miséricorde, in Caen, enables the oversight of the welcoming and the care of migrant populations as well as screening program for HIV and HCV by RDT and for tuberculosis by pulmonary X-ray. Various situations have been the beggining of an ethical reflection about autonomy and consent, through precaritie's situation but also public health. Ethical discussion is organized around three approaches: legal, social ans medical.
Conclusion: individual autonomy, especially during screening, is a principle wich can be the source of tension with public health. Consent is weakened by the language barrier and by the precarity of migrant population which doesn't make health a priority for them. After all, health professionals do all they can to ensure patient's comprehension and adhesion to care, in their interest with beneficense, wich they understand.
