Évaluation de la faisabilité et de l’acceptabilité des automesures tensionnelles nocturnes dans le cadre d’une action de soins pluriprofessionnelle, en médecine générale et en milieu rural

Abstract : Failure to reduce nighttime blood pressure would increase the risk of cardiovascular comorbidities, and Ambulatory Blood Pressure Monitoring (ABPM) is the gold standard for screening for nocturnal hypertension. However, the geographical isolation of some populations makes access to ABPM difficult, hence the interest in seeking alternatives. The main objective of our study was to evaluate the feasibility of day and night self-measurement of blood pressure in rural consultant patients (monocentric study in Gabarret) by analyzing the responses to questionnaires. The secondary objective was to evaluate the acceptability of these self-measures. According to our study, our protocol of day and night self-measurement with therapeutic education proved feasible and acceptable for the majority of patients, except for sleep, where acceptability was more moderate. However, our protocol could be improved on several levels (autotensiometer keeping night measurements in memory, larger population included, more disparate in age, profession and pathology). Future prospects would be that our protocol could be a preamble to more robust studies comparing ABPM to nocturnal self-measurement in order to provide an alternative for patients in rural settings remote from ABPM.
