Comment les médecins généralistes gèrent-ils les incertitudes liées à la délivrance des certificats d’absence de contre-indication (CACI) pour les sports sans contraintes particulières ? : étude qualitative par analyse thématique sur la base de 13 entretiens de médecins généraliste normands

Abstract : Introduction: Registration in a sports club or participation in a competition requires the patient to present a Certificate of Absence of Contraindications (CACI in French). For sports without specific restraints, this certificate can be issued by the general practitioner but the unwinding of this medical consultation is a current debate within the medical community. The purpose of our work was to identify the origins of this uncertainty related to the issue of these certificates and the behaviours adopted to manage them. Method: Qualitative study by semi-directed interviews with thirteen general practitioners delivering CACIs for sports without specific restraints. Results: The consultation’s objectives varied depending on the practitioners’ expectations. The act of delivering the certificate interpellated and was perceived as a way to shift the responsibility to the doctor. The doctor’s report to the notion of sudden death, the implication of responsibility, the variety of expert opinions the lack of confidence in the relationship and the discomfort generated by the highlighting of these uncertainties have been sources of cognitive dissonance among practitioners. However, practitioners manage these uncertainties by remaining attentive to their patients, by adapting their behaviour, by questioning the system and by being proactive. Conclusion: These uncertainties should encourage amendments to the CACI consultation, such as questioning screening policies and promoting patient empowerment.
