Évaluation qualitative de la psychothérapie transculturelle : expérience subjective de professionnels de santé travaillant auprès des adolescents et leur famille

Abstract : Multicultural societies bring a new complexity to the clinical psychiatry. Migrants present psychiatric disorders more frequently and access to care and management is less efficacy than for the host population. Thinking psychotherapy in relation to the cultural dimension becomes essential to respond to these difficulties. The transcultural psychotherapy (TCP) consultation was created to overcome this lack. Our objective is to evaluate the subjective experience of first-line therapists (FLTs) referring to their patients to transcultural consultations in order to better understand how an awareness of the transcultural approach can influence therapists. Method : semi-directed interviews were conducted with FLTs who referred their patients to the TCP in Paris. The data were analysed using a thematic qualitative methodology. Results: Five FLTs were interviewed. They referred their patients to the TCP when they were in front of a "block" in individuals’ follow-ups and without "resources" to take cultural elements in consideration. TPC allows the flow of speech through the group and the interpreter. FLTs, through a richer knowledge of the patient, can better understand him and improve their therapeutic alliance. More generally, the elements of TCP lead to a change in the position of FLT in relation to the patient. FLTs, in their clinical practice, learn to question patients about cultural factors and with the reopening of dialogue, they identify the central role of narrativity in understanding the patient's functioning, reluctance and difficulties. Conclusion : our results show how TCP represents an alternative to complex follow-ups to give a new dynamic to individual management and improve FLTs’ countertransference. We can also highlight how TCP allows evolution in the clinical practice of FLTs by developing cultural skills to intervene effectively with patients from diverse cultural backgrounds, through the improvement of their listening skills with the awareness of the importance of cultural aspects and flexibility of beliefs.
