Influence de l'avulsion de la troisième molaire mandibulaire incluse sur le parodonte de la seconde molaire mandibulaire : revue systématique de la littérature

Abstract : Third molar extraction is the most common oral surgery in France. Impacted third molar's avulsion requires a mucco periosteal flap and osteotomy, which may influence the healing of the distal periodontum of the second molar. The objective of this study was to do a systematic review in order to evaluate the evolution of the distal periodontum of the second molar after the impacted third molar surgery and if the flap design could have an influence on it. Studies were included from two databases, Pubmed and Scopus. The studies had to be clinical, longitudinal, published between 2010 and 2020 and had to evaluate the evolution of the distal periodontum of the second molar or compare two different flap design and their impact on the distal periodontum of the second molar. Twenty-two articles were included in this study. The parameters studied were the peridontal probing depht, the clinical attachment level, the plaque index, the gingival index and the bleeding on probing. The impacted third molar avulsion may lead in an improvment of the health of the distal periodontum of the second molar. Age and pre existence of periodontal pockets may be risk factors of a poorer healing. The flap design did not have an influence on the healing of the distal periodontum of the second molar. Other studies with higher evidence level will be required to confirm these results and to highlight other possible risk factors.
