Boiterie et douleurs des membres inférieurs d'origine non traumatique : analyse des pratiques aux urgences pédiatriques du Centre Hospitalier et Universitaire de Caen

Abstract : Limping and limb pain are common reasons for consultation in pediatric emergencies. The interrogation and clinical examination are sometimes difficult due to the young age of the child. The management must be rapid, but also exhaustive and systematic, so as not to disregard diagnostic or therapeutic emergency. However, the full examination is often not carried out by practitioners, due to lack of training during their course. We carried out a retrospective analysis of practices in pediatric emergencies at the Caen CHU over the first six months of 2014. Two hundred and thirty-three children were included. Results: Children under 3 years consult for limping. Fever is significantly more sought after in cases of limping. Examination of walking is not systematic. Axial and muscular examination are not performed. Biological examinations are performed when the clinical examination is not contributory. Joint ultrasound is mainly prescribed for limping and hip pain. No diagnosis is made in 46% of cases. The diagnosis of transient acute synovitis is the main one withheld, probably in excess, responsible for a lack of further invest. Children who revisit do so less than one month after the first visit. Few exams are prescribed remotely from this consultation. Conclusion: A protocol guiding the practitioner in the management of these consultations would frame the prescription of additional examinations and clarify the continuation of the management to be proposed.
