Parcours de soins des enfants atteints de troubles spécifiques des apprentissages à Paris : difficultés rencontrées et place du médecin généraliste

Abstract : Introduction: the objective of this study was to describe the care pathway of children with specific learning disorders (SLD) in Paris in 2019, the difficulties encountered and the place of the general practitioner.
Material and method: descriptive survey based on a questionnaire. This questionnaire was distributed to parents of children between the ages of 5 and 15 with SLD in 40 speech therapist's offices randomly selected in Paris.
Results: 131 questionnaires were analyzed. The average time between identifying the first difficulties and the diagnosis was 19 months (versus 4 years in France). The main difficulties encountered during the course concerned: the lack of availability of professionals for 36.15% of parents, the difficulty of setting up educational adaptations at school for 41.54% of parents and in particular the time it tooks to set up these adaptations for 30% of them, the lack of communication between the professionals for 34.62% of the parents, 19.23% of the parents encountered financial difficulties and 18.46% of them had difficulties identifying which professionals they should consult. The general practitioner didn’t appear much involved in the detection and diagnosis of the disorders, he stepped in nevertheless early in the course of care and he was quoted as referent for the follow-up by 23.08% of the parents.
Conclusion: the care path of children with SLD in Paris, although facilitated and accelerated by a better medical and paramedical demography, still presents many obstacles, many of which could be improved by a better involvement of the general practitioner.
