Allergie aux bêtalactamines : devenir des patients après exploration allergologique

Abstract : Introduction: Allergy tests are safe and effective to determinate if a patient is truly allergic or not. Our objective was to assess their benefit in practice by determining the rate of resumption of betalactams. Methods: We carried out a retrospective study by phone survey at the University Hospital of Caen. Each patient for whom reintroduction tests have been negative for at least one betalactam allowing its re-prescription was questioned on their resumption or not of this beta-lactam. Results: One hundred and sixty patients were included. Considering the 63 patients who required a betalactam since the tests, the rate of authorized betalactams resumption was 58.7%. Among the reasons for using another antibiotic, 9% of patients still thought they were allergic, 9% and 15% respectively declared apprehending or refusing to take the antibiotic again. Among patients who have taken an antibiotic since the tests, unauthorized antibiotic was prescribed for 15%. An urticaria and a purpura were also noted with betalactams authorized by the tests, without being able to confirm the immuno-allergic origin. Five percent of patients kept their allergy card despite negative tests, and 79.7% of regular pharmacies had no information about their patient's allergies. Conclusion: Better patient information and communication between healthcare establishment and general practitioners and pharmacy could help improve the compliance of tests.
