Les réactions cutanées bénignes chez l'enfant suite à la prise d'Amoxicilline sont-elles allergiques ? : prise en charge allergologique dans le service de pédiatrie à l'hôpital de Flers

Abstract : Introduction: Skin reaction after Amoxicillin intake are seldom allergic; when they happen in children, those children are often considered allergic all their life without testing. Primary objective of the study: amongst the children with benign skin reaction and who’ve had an oral challenge in Fler’s hospital; we seek to determine the proportion of them who suffers from actual Amoxicillin allergy. Secondary objectives: a litterature review on the diagnosis of Amoxicillin’s allergy, and to identify which doctors refer their patients to the hospital for assessment. Methods: Monocentric retrospective study on 24 children between 2 months and 17 years-old, who have had amoxicillin oral challenge at Flers’ hospital, from 2016 to 2019. Results: 24 children were tested; challenge for 8.4% patients (2 patients) came back positive. Diagnosis of allergy relies on skin testing, and oral challenge when skin test is negative. 7 (29.2%) patients were referred to the hospital by a private paediatrician; 5 (20.8%) by the hospital paediatrician, 6 (33.3%) by the hospital allergologist, 4 (16.6%) by their general practicioner. 2 (8.3%) patients had no data. Conclusion: Our study is consistent with litterature data on the subject, benign skin reaction following amoxicillin intake are seldom allergic. Oral challenge is the best test to achieve a correct diagnosis. General practitioners rarely refer their patients to allergologists at Flers’ hospital. For patient with low probability to be allergic to amoxicillin, oral challenge at home does not seem to be dangerous.
