Évaluation de l’utilisation de la téléexpertise chez les médecins libéraux et hospitaliers dans le territoire du GHT Léman-Mont Blanc

Abstract : Tele-expertise is changing our medical practice. The ceaseless improvement of communication technologies makes it appear as an attractive solution in the face of medical desertification, new economic constraints, and the growth and aging of the population. This new support for patient care as well as the creation of regional hospital groups aim to pool resources with the aim of equality and ease of access to care for the population. The objective of our work is to assess the proportion of use of tele-expertise by private and hospital doctors in the Léman-Mont Blanc GHT. The secondary objectives are to list the different tele-expertise tools used, to compare their use according to the socio-demographic characteristics of physicians and finally to report whether other means of communication are used between practitioners. We carried out a quantitative, multicenter study from October to November 2020 in the GHT Léman-Mont Blanc. Doctors were interviewed through a questionnaire send by mail. The doctors excluded were those who did not have a computer or did not belong to the territory of the GHT Léman -Mont Blanc. The response rate obtained to our questionnaire is 19.3%. The proportion of private and hospital doctors from the GHT Léman- Mont Blanc using tele-expertise is 48%. The main tools used are MonSisra for 84% of practitioners but also MyHCLpro and other platforms created by private companies such as IMADIS, Covotem, Telemedica or Xplore. Furthermore, 81% of private and hospital doctors questioned declared that they used unsecured messaging (email, telephone messaging, etc.). Finally, the use of tele-expertise depends on the specialty of the doctor. Tele-expertise remains a very common subject in the medical population. Most doctors have a pejorative opinion on this one, it is described as complex, time consuming. On the other hand, it is seen as useful and secure. In order to standardize practices and for equal access to care within the GHT, it would be useful for former physicians to use tele-expertise, particularly on MonSISRA. It would also be interesting to ensure benefits for the care of the patient.
