État des lieux de l’activité de don de sperme au CHU de Caen à l’aube de la nouvelle loi de bioéthique

Abstract : Introduction: While awaiting for the new bioethics law covering the activity of sperm donation, the main objective of this study was to perform an evaluation of this activity at the university college of Caen hospital over the past 20 years. The secondary objective was to identify prognostic factors for pregnancy and live birth in beneficiary couples. Methods: A retrospective analytical study was performed. We have analyzed donor files since 2000 as well as beneficiary files since 2014. Data was collected using registers, paper records as well as Medifirst® software. Results: Between 2000 and 2018, 236 men presented themselves to donate their sperm,152 were accepted of which 147 were included in our study and 84 were rejected. 98 accepted donors still have semen straws to allocate. 12 donors gave 10 live births and 28 didn’t give any pregnancy. The quality of sperm as well as the number of pregnancies per donor appear to have declined since 2000. Between 2014 and 2020, 431 cycles of IUI and 151 cycles IVF/ICSI were performed, the pregnancy and live birth rates per cycle were respectively 24.3% and 19% in IUI and 37% and 30% in IVF/ICSI. The determining factors in IUI beneficiary couples were the female age, the number of motile inseminated sperm, and the infertility of mixed origin. Discussion and conclusion: The results in our study are reassuring concerning the stock of donors, but the evaluation of the impact of bioethics law evolution on the number of donors and the number of requests for beneficiary couples, is to be estimated after its effective application.
