Étude évaluant l’efficacité du laser à colorant pulsé sur les télangiectasies du visage et du décolleté des patients suivis pour une sclérodermie systémique

Abstract : Background and objective: The pulsed dye laser (PDL) is the reference in the treatment of vascular lesions (1). Few recent studies have been performed in patients with systemic scleroderma (ScS) (2-4). However, cutaneous telangiectasias (TA) are one of the cutaneous features of SSc and have a conspicuous character. Our objective was to evaluate the efficacy of PDL on TA of the face and décolleté in these patients. Material and methods: We performed a monocentric and prospective study at the Caen University Hospital. 18 patients performed 4 PDL sessions spaced 2 months apart. They were followed from September 2019 to June 2020 at the CHU of Caen and were all suffering from SCS. Our primary endpoint was the decrease in the average number of TA. Our secondary endpoints were the evaluation of the number of BPs by an independent observer, the calculation of a T-Score, the evaluation of quality of life, the aesthetic evaluation by the patient and by an adjudication committee, the evaluation of pain and adverse events (AEs), and patient satisfaction. Results: After 4 PDL sessions, we observed an average decrease of 6 BPs. The T-Score was significantly decreased. 100% of patients reported good or improved. Pain induced by the sessions averaged 3.5 out of 10 using the Visual Analog Scale. 50% presented AEs that were generally transient and tolerated. Conclusion: PDL is effective in the treatment of TA in ScS patients. Physical and psychological improvement and patient satisfaction make PDL an essential treatment to propose today.
