Évaluation des attitudes et pratiques des médecins généralistes libéraux sur la mise en place du programme de dépistage organisé du cancer du col de l’utérus et son impact dans leur pratique quotidienne

Abstract : Introduction: cervical cancer can be considered « avoidable » through vaccination and vaginal smears. The slowdown of beneficial effects of these prevention measures and a stable and low national coverage rate for patients eligible for pap smears have led to a new organised screening in Nouvelle-Aquitaine through the summer of 2020. The main objective of our study was to evaluate the behaviours and practices of private general practitioners in the departments of Gironde and Pyrénées-Atlantiques on the implementation of the cervical cancer organised screening and its impact on their daily practice. Methods: we performed a qualitative study through individual and semi-guided interviews on 20 private general practitioners in Gironde and Pyrénées-Atlantiques. Results: even though most general practitioners had not read the letter of information at the time of the study, they seemed to welcome the screening fairly well. They sense a probable increase in screening promotion by sending letters and reminders at the patient’s home, by simplifying the process, by networking about screening and being more actively involved, and by systematically receiving their patient’s test results. However, there still are some obstacles: doctor-patient closeness, insufficient training, lack of time or complicated planning, fluctuating involvement and commitment from doctors, lack of equipment and weakness of the network. Some tools were suggested to face these obstacles: self-induced swab, promote screening, better training since internship, reorganisation of the practice, increase in salary. Conclusion: the implementation of cervical cancer organised screening is well accepted by the general practitioners in Nouvelle-Aquitaine. However, a lot of practitioners do not feel like they will take a bigger part in the screening by only keeping the pap smears in place. It will be necessary to implement new practical measures.
